I hereby apply for an Ambyr Club membership. If accepted, I agree to be bound by the rules of the club.

All information shall be treated in confidence.

Ambyr Club’s current membership dues and initiation fees are as follows:

• Gold Membership dues are $99 per month or $265 per three months, plus a one-time initiation fee of $199. Platinum Membership dues are $129 per month or $350 per three months, plus a one-time initiation fee of $199.

• Both membership dues and initiation fees are subject to applicable taxes and service fees.

• Both membership dues and initiation fees must be paid within seven calendar days of being accepted and before gaining access to the club. Failure to do so will result in a terminated application and membership. You will not be permitted to apply again.

• Your Membership will auto-renew on an monthly basis or every three months depending on your payment plan. If you wish to cancel your membership, you must provide notice 14 days prior to renewal.

This form will take around ten minutes to complete.

If you are selected for a virtual interview, you must complete your interview within thirty days of notice. Failure to do so will result in a terminated application and membership.
